**What happened:** Assaf reviewed the Revolut page and flagged it as "so bad." On inspection: broken/empty image areas throughout — agent had pulled 100+ CDN assets, many not rendering.

**Root cause:** Shipped 17/17 without visual screenshot check. Never verified eyes-on before reporting done.

**Actions taken:**
- Spawned revolut-rebuild agent to fix broken images
- Spawned visual-qa-all agent to screenshot all 17 pages and produce qa-report.md
